This office is not responsible for providing the MS Excel 2000 software or the technical support that most software vendors provide. For information on using Microsoft software, refer to the Help Section of your software program or visit the Microsoft Web site.
- Download the MS Excel File
Campaign Contribution Disclosure Report (MS Excel)
Start by clicking on the document link. If the MS Excel software is installed on your computer, once you click on the document link the file should open in MS Excel. If you do not have the program, you will be given an option to save the file and will need to install the MS Excel software.
Once the file has opened in MS Excel you can begin completing the CCDR.
- Complete the CCDR / Save a Blank Copy for Future Uses
You may want to save a blank copy prior to beginning your report for future use or in case you want to start again.
Review your report and check your entries prior to sending.
- Email Your Report
Once you have completed the report, save the MSExcel file using the file format listed below.

Email the CCDR report as an attachment to elections@co.clayton.ga.us.
In addition to filing electronically you MUST ALSO print, have the CCDR notarized and either mail or hand deliver to the address below:

Required E-Filing of Disclosure Reports for Candidates Raising or Spending a minimum of $10,000 in an election cycle (Code Section 21-5-34.1(c)) - Effective January 1, 2003, candidates seeking election to county or municipal offices shall use electronic means to file their campaign contribution disclosure reports with the election superintendent of their county or the municipal clerk or chief executive officer of their municipality, as specified in O.C.G.A. § 21-5-34, upon having raised or spent a minimum of $10,000 in an election cycle, but contributions and expenditures received or made prior to reaching such threshold need not be electronically filed if previously reported, except as cumulative totals. Under that threshold, electronic filing is permitted and encouraged but not required.
