Vital Records - Birth and Death Records

Only records for events occurring in Clayton County are available from this office. A fee of $15.00 is charged for the first certified copy obtained. Additional copies may be purchased at the same time for an additional fee of $5.00 per copy.

There are two (2) methods of obtaining vital records, in person and by mail.

When applying in person, you must present proper picture identification. The court will accept a valid driver's license, a state issued identification card, military identification or a valid matricular consular card. Method of payment is by cash only; checks are not accepted.
When applying by mail, payment must be in money order form payable to Clayton County Vital Records. You must include the following information; the registrant’s name, date of birth, place of birth, father’s name as it appears on the certificate and the mother’s full maiden name. Also required is a copy of the requestor’s identification and a self addressed stamped envelope. The mailing address is Vital Records, 121 South McDonough St., Jonesboro, GA 30236.

Clayton County Vital Records is located in the Probate Court at 121 South McDonough Street, Annex Building 3, First Floor, Jonesboro, Georgia. Office hours for obtaining vital records are Monday - Friday, 8:00 am until 4:30 pm, excluding holidays.
